As per Federal Law drivers of commercial motor vehicle need to have a DOT (Department of Transportation) physical.

Our providers are both licensed as a “medical examiner” listed on the Federal Motor Carrier Safety Administration National Registry (FMCSA) and may conduct a Department of Transportation (DOT) physical.

It’s easy to get your DOT physical done at Ideal Family Practice. Simply schedule your appointment. Arrive 15 minutes prior to your appointment to complete paperwork. You can go to the FMCSA website to access the new forms (effective April 20, 2016) and see related FAQs. Forms are available at Ideal Family Practice. Remember you will need to provide a urine sample so make sure you will be able to void once at your appointment.

Also please bring with you a photo ID, other test results that may be required, glasses, hearing aids, and a complete list of your medications and prescriptions.

After your physical we will finish the necessary paperwork. Please note that you must pass ALL DOT requirements to be cleared for your CDL medical card. According to FMCSA the new regulations do not allow for certification extensions and if you do not pass any part of the exam you will be required to have a complete new exam in order to become medically certified. By Federal Law Medical Examiners are not allowed to give a driver an “Incomplete” status or “Determination Pending” status.
