Routine appointments
Physicals, annual gynecological (GYN) exams, and non-urgent health concerns (e.g., skin problems, back or joint pain, sleep problems, and medication refills) are considered routine medical appointments.

Same day appointments
For more urgent health concerns that cannot wait for a routine appointment, no appointment is necessary. We accept walk-in visits. You you prefer you can always call us to schedule a same day appointment with one of our providers for a medical issue that cannot wait.

Discharge from a hospital or rehab facility
If you have been discharged from a hospital or a rehab facility it is important that we see you as soon as possible. Call our office for assistance.

Specialty care appointments
For an appointment for any of our specialty medical services, please call ahead to schedule an appointment at a convenient time. Specialty medical services include allergy testing, deep tissue laser therapy, laboratory services, immunizations, vaccinations, etc.

Emergencies
If you are experiencing an emergency, please call 911 immediately or go to the nearest emergency room. Please call the office (even after hours)  and let us know if you are in the hospital or a rehab facility.

Be sure to call Ideal Family Health if you are unable to keep an appointment. A fee maybe charged for missed appointments that are not canceled twenty-four hours in advance.

Things to bring to your appointment:

If you are a new patient, please arrive 30 earlier then your scheduled appointment to complete any necessary paperwork.  You can download the new patient registration packet right from our web site.

On the patient registration paperwork there is a section to complete if you would like us to discuss your health information with someone (a parent, child, relative, guardian, partner, or friend). Remember, we do not share any of your personal health information with friends or family without your permission.

Please bring your driver’s license, insurance card and form of payment.

Co-pays, deductibles and outstanding balances are due at time of service.
Forms: Please bring your completed paper work.

List of all medications: Please bring a list of medications you are on including name, dose, how often you take it.

If you are not feeling well: Please make a list of your symptoms – include when they started and any other details such as how they have progressed, and any changes you have noticed.

Be prepared to answer a series of questions about your health history, allergies, habits, and use of medications, etc.

Feel free to ask questions about your symptoms, follow-up care, side effects of treatments or medications, or any other concerns.
